This policy includes coverage of $1,000,000 per occurrence and $2,000,000 aggregate liability, with the Dougherty County School System (200 Pine Avenue, Albany, GA 31701) listed as the “Certificate Holder” and “Additional Insured.” I understand that this coverage applies to sanctioned team activities but does not replace personal medical insurance for individual injuries or incidents.I grant Albany Basketball Club permission to use my child’s name, photos, or videos for promotional materials, social media, and marketing purposes.I acknowledge that tryout fees (if applicable) are non-refundable and do not guarantee a spot on the team.I have paid my $15.00 tryout fee through the Zeffy form. The tryout fee is required for participation.
